BLOOD TRANSFUSION
CONFIRM ORDER
-Check the pt's record & confirm the provider put the order for a blood transfusion -also confirm Type& screen results are done and documented (Blood Type) 1 hr prior - Consent form has been signed
GATHER SUPPLIES
-Biohazard bag -alcohol pads -blood tubing(Y-tubing) -blood product -500cc bag of Normal Saline -10 mL normal saline flush
TWO NURSES MUST VERIFY
-blood before the start of the procedure, -verify blood band width blood unit, -verify blood unit with the request form with the nurse. *make sure all matches and info is correct
GO TO THE PT'S ROOM CONFIRM CONSENT IS SIGNED EXPLAIN PROCEDURE
Ok so Blood is compatible -Provide privacy -Verify name and B-day with wrist band -You are aware the doctor order a blood transfusion for you -Do you have any allergies -Do you have any questions about the procedure. -I see that you have signed the consent already -Ok so let’s go ahead and start
STANDARD PRECAUTIONS
Wash hands Don gloves -Take vitals
THE PROCEDURE
-Ensure patency of IV line, (flush Line) -hang normal saline bag, -spike normal saline bag and prime tubing - fill blood tubing filter completely with the saline. -Caution: close normal saline roller clamp before next step.
STEP 1 PRIME AND PREP Saline
STEPT 2 PRIME AND PREP Blood
- hang & Spike blood bag -Open blood roller clamp and prime tubing. *Be sure to prime the entire line. -Connect IV tubing to patient's IV access. -Set pump to deliver blood at no more than 2 milliliters per minute, for 15 minutes.
-Open port at IV site and begin transfusion
MONITOR PT
* do not transfuse for more than 4 hrs
INFUSION COMPLETE
-Monitor patient for adverse reactions. -Measure vital signs after 15 minutes to ensure tolerance of blood. -If no Adverse reaction: Increase infusion rate after 15 minutes per MD order or facilities policy.
-Measure vital signs when infusion is complete. -Disconnect blood tubing from IV site. -Flush IV line wit NS -Observe patients response to transfusion. -Dispose of soiled supplies, tubing and blood bag, and gloves -In appropriate container
ENSURE SAFE ENVIRONMENT
- Return bed to lowest height with brakes locked -side rails up and call light in reach. -Wash hands for CDC guidelines". -Document procedure And exit the room
Priority action: stop transfusion, and open NS, notify provider Coinfirm blood products and patient information is correct, to ensure no mistakes were made -Return remaining blood to blood bank Transfusion reaction: Mild: *fever increase by 2 or more degrees = reaction S/S: fever chills, Hives/itching, nausea and anxiety. give antihistamine(benadryl) Adver effects "NEED TREATMENT"-Anaphilactic: chest tightness, dyspnea , low BP, High fever, red urine Give IV epinephrine/steroids/antihistamines & Bronchodilators...
