Bukidnon Provincial Medical Center During our orientation in Bukidnon Provincial Medical Center, I have learned several things. Some are just reviews because some protocols are just almost the same with other hospitals specifically the covid 19 protocol. The only difference is that in Bukidnon provincial medical center, they are the main institution where the actual or active covid 19 patients are being admitted all around the entire bukidnon. We were oriented first with their management and handling of patient in hospital during this trying time. We were oriented that upon entering the institution the patients are being asked for their rt pcr test result. Also disinfection at all times is also observed. We were also oriented that all of their staff nurses are wearing their own protective equipment at all times. The institution is being divided by floor and area. 3rd floor is where they place the actual covid 19 patients and second floor is where those non respiratory problem patients such as for surgery or vehicular accident patients are being admitted. And no one is allowed to go on third floor aside from staff nurses on that floor. No visitors are allowed inside the institution. For outpatient department, they are no longer operating on that area because they do not want out patients’ contract the virus given that they are the main institution taking care of the active cases of covid 19 virus. This was being strictly followed to avoid contamination among other health workers and civilians. We are also oriented with their house rules and shifts of staffs and their tasks specifically for the students’ rules and roles. Their shifts would be 8 hours per shift starting from 7 am to 3 pm, 3 pm to 11 pm, and 11 pm to 7 am. If the shifts is 11 pm to 7 am, double red line will be drawn on the chart before start charting for the following date. It was mentioned that the students should always asks their instructor first before doing tasks. Regarding the tasks, some competencies are allowed to be done by the student if the instructor will approved and will assist the students. We were also oriented with the attitude and manner of the staffs inside the institution. Always be courteous kind and respectful to everyone on the floor. They do not accept the reasoning such as “Ingana najud akong tingog maam” because according to them, some who reasons like that becomes a soft spoken if speaking with the higher ups or supervisors so it was emphasized. In giving medication all medication must be checked by the CI was emphasized before allowing the students to administer the due medication. There should always be a medication ticket that is prepared. The medication ticket should have the name of the patient, the name of medication, the exact time of medication administration and room or ward. They have white ticket which is once a day, yellow for twice a day, pink for 3 times a day, blue for 4 times a day. Before administration double check for the patient history of allergies, inform or teach the patient about the type of drug to be administered. Also it was emphasized to be wary of possible drug to drug interaction. It was also emphasized during orientation the prior to administering the medication always observed the 12 “R” of drug medication the right patient, right drug, right dose, right route, right time, right assessment, right approach, right of client to refuse, right of the client to know the reason why that specific drug is necessary, right evaluation, right documentation, right drug preparation. In documentation also there was a specific arrangements of the patients paper inside their folders it depends upon the institution what are their paper arrangement in documentation, when doing a nurses notes it should be clear and concise and should not be scribbling to avoid error. Carry out the doctor’s order legibly and properly and always clarify with the doctor and confirm any doubt on the doctor’s order to avoid errors and transcribe the doctors order accurately and completely. If error was committed in writing the chart, Using correction fluid or tape is prohibited. Instead place the wrong entry in an open

and close parenthesis and draw a straight line across the error entry and affix the signature. This should signify that there was an error and your signature will determine that you are the one who committed the error and had it corrected....
