Mr. Bleau decided not to take his dose of Toprol XL, a beta-blocker known to help alleviate angina, heart failure, and blood pressure abnormalities. After the exercise physiologist provided the opportunity for informed consent, Mr. Bleau began the Bruce Protocol on the lowest level of the test—1.7 miles per hour on a 10% gradient. He seems to have stable angina only after quitting the test at the eight-minute mark, which is within Stage Three of the Bruce Protocol. This is a potential indicator of several scenarios: Mr. Bleau’s Rating of Perceived Exertion is recorded around the 12-20 level; he experienced angina before or around the same time that he decided to quit; he potentially stating a three or a four on the angina scale; and his very high maximum blood pressure of 240/120 suggests a high susceptibility to stroke or a myocardial infarction due to being off of not only Toprol XL, but all of his medications and especially nitroglycerin. Further, angina that is either moderate or severe is an absolute indication to stop the Bruce protocol....
