PURPOSE OF MEDICATION

Expected Pharmacological Action Produces CNS stimulation by releasing norepinephrine from nerve endings.!

Therapeutic Use Increased motor activity and mental alertness and decreased fatigue in narcoleptic patients. Increased attention span in ADHD.

Complications CNS:!hyperactivity,!insomnia,!restlessness,!tremor, behavioral disturbances, depression, dizziness, hallucinations, headache, irritability, mania, thought disorder.!CV:!palpitations,!tachycardia, arrhythmias, hypertension.!GI:!anorexia, constipation, cramps, diarrhea, dry mouth, metallic taste, nausea, vomiting.!GU:!erectile dysfunction, increased libido.!Derm:urticaria.!Misc:!physical dependence, psychologic dependence.

Medication Administration PO, tablets

Contraindications/Precautions Hypersensitivity to sympathomimetic amines, glaucoma, agitated states, advanced arteriosclerosis, symptomatic heart disease, moderate to severe hypertension, hyperthyroidism, history of drug abuse

Nursing Interventions Monitor growth rate closely in children. Interrupt therapy or reduce dosage periodically to assess effectiveness in behavior disorders. Tolerance to anorexiant effects may develop after a few weeks

Interactions Drug:! Acetazolamide,!sodium bicarbonate!decrease dextroamphetamine elimination;!ammonium chloride,!ascorbic acid!increase dextroamphetamine elimination; effects of both!BARBITURATES and dextroamphetamine may be antagonized;!furazolidone!may increase BP effects of!AMPHETAMINES

Client Education Evaluation of Medication Effectiveness - Monitor attentiveness and behavior in pts with ADHD - Be aler for excessive CNS simulation - Assess heart rate, ECH nd heart sounds, BP - Assess dizziness that migh affect finctional activities

ACTIVE LEARNING TEMPLATES

Swallow sustained release capsule whole with a liquid; do not chew or crush. Do not drive or engage in other potentially hazardous activities until response to drug is known. Taper drug gradually following long-term use to avoid extreme fatigue, mental depression, and prolonged sleep pattern. Do not breast feed while taking this drug.
