Expected Pharmacological Action Rifampin is bactericidal as a result of inhibition of protein synthesis.

Therapeutic Use - Rifampin is a broad-spectrum antibiotic effective for gram-positive and gram-negative bacteria. - Rifampin is given in combination with at least one other antituberculosis medication to help prevent antibiotic resistance

Contraindications/Precautions Use cautiously in clients who have liver dysfunction Nursing Interventions

Interactions

- Monitor liver function Administer oral rifampin 1 hr before or 2 hr after meals. Absorption is decreased if given with food

- Rifampin accelerates metabolism of warfarin, oral contraceptives, protease inhibitors, and non-nucleoside reverse transcriptase inhibitors (NNRTIs) for HIV, resulting in diminished effectiveness Concurrent use with isoniazid and pyrazinamide increases risk of hepatotoxicity - Avoid alcohol consuption

Client Education Evaluation of Medication Effectiveness - Improvement of tuberculosis manifestations, such as clear breath sounds, no night sweats, increased appetite, and no afternoon rises of temperature - Three negative sputum cultures for tuberculosis, usually taking 3 to 6 months to achieve
