Chronic stable angina, vasospastic angina; to reduce risk of hospitalization of angina; to reduce risk of coronary revasularization procedure in patients with recently CAD by angiography and without heart failure. Hypertension

Inhibits calcium ion influx across cardiac and smooth-muscle cells, dilates coronary arteries and arterioles, and decreases blood pressure and myocardial oxygen demand.

Significant Labs

Route/Dosage/Frequency For angina: - Adults: initially, 5-10 mg PO daily. - Elderly: initially 5 mg PO daily For those who are small or frail or have hepatic insufficiency, initially, 5mg PO daily. For hypertension: - Adults: initially give 5 mg PO daily. Dosage adjusted to patient response and tolerance. Titration should occur over 7-14 days. Maximum daily dose is 10 mg. - Children 6-17 years: 2.5-5 mg PO once daily. Maximum dose is 5 mg daily. - Elderly: initially 2.5 mg PO daily.

Side Effects/Adverse Reactions -

CNS: headache, somnolence, fatigue, dizziness Cardiovascular: edema, flushing, palpitations GI: nausea, abdominal pain Respiratory: pulmonary edema, dyspnea Skin: pruritus, rash Overdose symptoms: marked peripheral vasodilation with hypotension and possibly reflex

Contraindications -

-

Contraindicated in patients hypersensitive to drug. Use cautiously in patients receiving other peripheral vasodilators, especially those with severe aortic stenosis or hypertrophic cardiomyopathy. Because drug is metabolized by liver, use cautiously and in reduced dosage in patients with

tachycardia.

severe hepatic disease. Use in pregnancy only if potential benefits justifies risk to fetus. Drug appears in human milk. Patient should discontinue breast feeding or discontinue drug.

Nursing Implications/Patient Education -

-

Monitor patient carefully. Some, especially with severe obstructive CAD, can develop increased frequency, duration, or severity of angina or acute MI after initiation of calcium channel blocker or at time of increase of dose. Monitor blood pressure frequently during initiation. Gradual onset of vasodilation can cause acute hypotension, yet rare. Notify provider if signs of heart failure occur. Give without regard for food. Abrupt withdrawal can cause increase of frequency and duration of chest pain. Taper dose gradually. Tell patient to report adverse reactions and to continue taking medicine even when feeling better. Tell patient SL nitroglycerin may be taken when angina symptoms are acute....
