Albuterol Drug Classification: Bronchodialater/ adrenergics (accuneb, proair hfa, Proventil hfa, Ventolin hfa)

Purpose of Medication: (Include 1. Pharmacological Action and 2. Therapeutic use and number them with 1. and 2.) 1.acts on beta-2 adrenergic receptors to relax the bronchial smooth muscle . Also inhibits the release of immediate hypersensitivity mediators from cells, especially mast cells. 2. Relaxation of airway smooth muscle with subsequent bronchodilation, prevents and treats difficulty breathing, wheezing shortness of breath, coughing and chest tightening.

Complications: Nervousness Tremors Headache Insomnia Chest pain Palpitations Angina Hypertension Nausea Vomiting Hyperglycemia

Medication Administration:  

Inhalant (adults) 2 inhalations every 4-6 hr

Exacerbation (adult) 4-8 puffs every 20 min up to 4 hours then 1-4 puffs as needed PO (adults) 2-4 mg 3-4 times daily not to exceed 32mg/day or 4-8mg extended release tab twice daily Nebulization (adults) acute asthma exacerbation 2.5-5mg every 20min for 3 doses then 2.5 – 10mg every 1-4 hr as needed





Contraindications/Precautions:      



Cardiac disease Hypertension Hyperthyroidism Diabetes Glaucoma Seizure disorder Pregnant women

Nursing Considerations: Asses lung sounds, pulse and bp before administering and during medication peak Note amount, color and character of sputum Monitor pulmonary function test before starting therapy and periodically throughout Observe for wheezing if it occurs withhold medication and notify physican

   

Interactions:  Concurrent w/ other adrenergic agents will increase effects  MAO inhibitors may lead to hypertensive crisis  Beta blockers may negate therapeutic effect  May decrease digoxin levels  Cardiovascular effects potentiated in patients receiving tricyclic antidepressants  Hypokalemia increase when using digoxin & tricyclic antidepressants

Evaluation of Medication Effectiveness:    



Prevention or relief of bronchospasm Long term control of asthma Clear lung sounds Absence of wheezing Baseline respiratory rate

Client Education:      

Take as directed / do not double dose or increase frequency Contact physician if shortness of breath is not relieved or is accompanied by diaphoresis, dizziness, palpitations or chest pain Prime unit w/ 4 sprays before using Discard canister after 200 days Consult with physician about al; Rx and otc medications before start of treatment Rinse mouth with water after each dose to minimize dry mouth...
