Assignment #2 - Teach a skill: Enema Enema Used as a therapeutic treatment to relive constipation. This can be given rectally. There are four different types of enemas: Cleansing Enema is used to remove feces using four types of solutions: Hypotonic (tap water), Isotonic (normal saline), hypertonic and Soupsuds(tap water with soap). IT stimulates the bowels to quickly expel both solution and impacted fecal matter. Carminative Enema is to provide relief from gas distention by instilling the solution through the rectum. This will release gas, which, in turn, distends the rectum and the colon which then leads to peristalsis. Oil Retention is used to soften the feces and to lubricate the rectum and anal canal, therefore, allowing passage of the feces by absorbing the oil. Return Flow Enema is used to expel flatus by repeating the process five to six times in order for the flatus to expel and the abdominal distention is relieved. Procedure: - Remember 3 checks and 10 rights - Check medical records and history - Perform hand hygiene prior to meeting with the patient - Identify patient through two identifiers - Inform patient about the procedure and ask for consent. - Provide privacy and adjust bed height in a comfortable level - Assess patient’s mobility, normal and last bowel movements and pain. - Bring the right equipment’s and materials - Perform hand hygiene - Prepare the solution with warm water

Add enema solution depending on which enema is being used. (In this procedure, I will be using a small bottle of liquid medication) Position patient on their left side in a Sim’s position (depending on patient’s comfort level and physical condition) Place disposable absorbent pad underneath patient and put on gloves Place a generous amount lubricant and on the tip of the enema bottle Use none dominant hand to separate the buttocks cheeks of the patient Ensure there is no abnormality. However, if there is a sight of bleeding or hemorrhoids – contact the physician right away. Ask the patient to take a deep breath to relax their muscles Insert the tip of the enema into their rectum Squeeze the bottle to start the flow of the fluid and roll the bottle up from the bottom to ensure that all the liquid is in (remember not to pump the bottle as it can cause the fluid and other components from their rectum to insert back unto the bottle) Remove the bottle and throw it out. Wait for the enema to take effect. This can take anywhere from 2 minutes to 1 hours (the kit would usually provide instructions with a specific estimate) Assist client, if needed, to go to the washroom or use the commode/bedpan to empty their bowels and identify the contents through its characteristics, colours and odour.
