NURSING SKILL WORKSHEET NAME: _________Amber Sias________ DATE: __09/30/21____ SKILL: _____________________

Describe Skill (short description of skill procedure)

Administering a cleansing enema proper with the most care and comfort for a patient.

Nursing Implications

Indications (Why is this procedure ordered?)

(Nursing considerations before, during, and after the procedure)

Enemas are used to treat constipation or cleansing out the bowel before diagnostic procedures or certain types of abdominal surgery. Preoperative enemas are for some surgeries.

Ensure the patient has privacy. Make sure that a commode or toilet is nearby because inserting an enema often gives the patient urgency to defecate. Lubricate the tip of the rectal tube for easy insertion. Encourage the patient to empty bladder as fluid entering the rectum may cause discomfort to an already full bladder.

Evaluation (Expected outcomes)

Stool is cleaned out. The enema turns clear. Abdomen is flat and nontender, with no distention.

Potential Complications (Describe potential problems when performing the skill)

Abdomen becomes rigid and distended. (pain/discomfort) Hemorrhaging occurs. Patient is having a hard time keeping enema stable. There is still feces being passed through after three large-volume enemas.

Patient Teaching (Describe what will be taught to the patient or family)

Before

Make sure patient is assessed and observed before starting. Instruct patient that enemas are NOT to treat cause of constipation. Instruct in daily life changes to support peristalsis.

During Instruct patient in selfadministration. Patient needs to lie in dorsal recumbent position with knees and hips flexed toward chest. Inform the patient about the potential side effects of the enema.

After Record the type, volume of enema given, time of administration, characteristics of the results, and the patient's tolerance of the procedure. Encourage questions and answer them....
