Step 1: • Gather a group of people that are knowledgeable about the problem for a brainstorm process Step 2: • On a board, draw a large skeleton of a fish Step 3: • Enter the problem statement at the fish head ex: missing usb cable in packing box Step 4: • Start brainstorm in 1 category on the potential causes ex: category: Man • List down all possible causes by man o Ex: new employees, do not understand the procedure o Inspector didn’t check o Damaged cable removed Step 5: continue brainstorm on the next category ex: category: machine • Ex: sensor malfunction • Cable stuck at dispenser • Drop outside box Step 6: repeat for the rest of the categories
Depending on the nature of the problem, these 5 categories might be different • Manufacturing problem man, machine, method, material, environment • Marketing problem people, packaging, pricing, promo, product, location • Service Problem suppliers, skills, safety, geographical, product After the fishbone had been filled up, go through each item with team member and discussed whether they can cause the problem or not once confirmed, get the team brainstorm on the solution for that root cause Benefits of using fishbone diagram are as follows: 1.
