Answer questions on the following scenario: Mrs. Dee an 86-year-old female on your unit who has had difficulty taking her pills. During report the nurse reported she pulverized Mrs. Dee's meds and administered them with applesauce, however, Mrs. Dee had trouble swallowing the medication. You are responsible for reporting Mrs. Dee's health needs at the interdisciplinary team conference. 1. As a team member, how would you advocate for this client? a. As Mrs. Dee’s nurse and a member of her care team, if I witnessed her having difficulty swallowing her pills even when they were crushed, I would be concerned about her choking and possibly aspirating. In order to try and solve that potential problem, I would express my concerns to the other members of the care team to try a different method of medication, such as liquid medications. Additionally, I would suggest a possible speech/language consult to determine the patients swallowing ability and if she may need thickened liquids to help. 2. What are the most important issues that need to be addressed with the other health care team members? a. In this situation, the most important issue would be to address the difficulty with swallowing oral pills because of the choking risk. As an offshoot of this issue, another issue becomes if it would be possible to give the patient liquid medication or not, which would be where the pharmacy care team member could help. A long-term issue of this would be planning medications for Mrs. Dee long term, as well as any other difficulties that she might have with this swallowing problem. 3. How would you convince a physician who is reluctant to make changes in the patient's plan of care to provide an order for liquid medications? a. If we were in a care conference, I would ask the opinion of the speech/language therapist and see if they would recommend a change in medication. Perhaps, if Mrs. Dee has not had issues with swallowing pills in the past, the doctor could prescribe liquid medications for a certain time and then try one dose of pills to determine if she is able to swallow whole or crushed medications again. Overall, the goal of the care team, especially if Mrs. Dee will be going home, is going to be finding a course of medication that the patient is able to take successfully so that she will continue to do so when she returns home, and minimizing the risk for the patient choking or aspirating any materials while struggling to swallow medications. 4. State 3 possible goals of this interdisciplinary team. a. Determine the best way for the patient to take medication without difficulty, and that the medications that the patient takes are able to be crushed or given as liquids. b. Evaluate the patient for any needed swallowing adaptations, such as liquid medications or thickened liquids. c. Finding a route for medication administration that will serve the purpose of the prescription while being the best option for the client to adhere to the prescription and schedule....
