HCM 320 Milestone One – Topic Selection

1. Public Health Issue Topic: Social Determinants of Health Social determinants of health can be defined as a combination of factors that contribute to health disparities and interfere with the lifestyle or living of people. Some of these are education, housing, poverty, safety, neighbourhood and built environment, that affect people socially, economically, environmentally, and even stressfully. Stress can stem from a result of lower socioeconomic status (Anciaes, P. 2019).

2. Economic Forces: There are economic forces that contribute to social determinants of health in communities. The lack of proper educational institutions, public health facilities, proper infrastructure (good roads, recreational parks, etc.), and public transportation could be considered macroeconomic forces that could contribute to social determinants of health. Microeconomic forces could be the absence of a proper job, eating meals with little or no nutritional value, lack of affordable housing, an inactive lifestyle, and stress.

3. Key Regulation Policy: According to the Kaiser Family Foundation (2018), the Center for Medicare and Medicaid Innovation (CMMI), established by the Accountable Care Act, announced the Accountable Health Communities (AHC) model in 2016. This initiative “focuses on connecting Medicare and Medicaid beneficiaries with community services to address health-related social needs” (2018). The CMMI works through State Innovation Models Initiative (SIM) to create a “multi-payer delivery and payment reform that include a focuses on population health and recognize the role of social determinants” (KFF, 2018).

4. Benefits and Consequences: Kaiser Family Foundation (2018) states that “a broad range of initiatives have been launched at the federal and state level within the health care system, including efforts within Medicaid to reflect new funding and demonstration authorities provided through the ACA to address social determinants of health and further health equity” (Kaiser Family Foundation, 2018). They are also looking at ways to enforce and expand work requirements in public programs, which they claim, is designed to the health and well-being of Medicaid enrolees and empower them to come out of poverty and attain independence (2018). On the other hand, the “the Trump Administration has started the process of pursuing policies that may prevent individuals in need from accessing programs that help to “address health and other needs and reduce resources to address social determinants” (Kaiser Family Foundation, 2018).

5. Impact of Regulation or Policy: Government policies and regulations can have impact on social determinants of health by supporting companies that share the same interest in helping to fight SDOH. The Medicaid program, Delivery System Reform Incentive Payment (DSRIP) that “emerged under the Obama Administration as part of Section 1115 Medicaid demonstration waivers” (Kaiser Family Foundation, 2018) is one such program. This regulation connects Medicaid funds and DSRIP waivers to projects that allow eligible providers to process performance metrics, which may involve addressing social needs. One example of the DSRIP working to combat the social determinants of health is in the state of New York. The state invests a significant amount of state dollars outside of its DSRIP waiver in housing stock to ensure that that a better supply of appropriate housing is available for those who qualify (Kaiser Family Foundation, 2018).
