Uses: Used to keep a normal balance of bacteria in the gastrointestinal tract. Helps to treat patients with irritable bowel syndrome, ulcerative colitis, pouchitis, and hepatic encephalopathy.

Nursing Considerations (what do you need to know when giving this med): Assess whether patient is also taking antibiotics, as antibiotics may inactivate the live bacteria in Visbiome. Do not give Visbiome within 4 hours after taking antibiotics. _____________

Side Effects: _Signs of an allergic reaction, such as a rash, hives, trouble breathing, trouble swallowing or swelling of the mouth, dizziness, fatigue, thirst, muscle pain, bone pain, weight loss, bloating, stomach cramps, or headaches. TO BE COMPLETED ON ALL MEDS WHETHER YOU ARE SCHEDULED TO GIVE THEM OR NOT. You MUST document how you will assess medication effectiveness, even if the medication was not given. Date/ Client initials
