Migraine Headache: characterized by recurrent episodic attacks of head pain that serve no protective purpose - Described as throbbing and unilateral - Triggering factors: caffeine, red wine, stress, MSG – each patient is different Assessment: *Aura: sensation that signals the onset of a headache or seizure; occurs immediately before migraine - With aura: - Without aura: most are without aura - Atypical: less common and include menstrual and cluster migraines - Phases: Prodromal, aura, headache, termination - S/S: unilateral, frontotemporal, throbbing pain often worse behind eye or ear, often accompanied by sensitive scalp, anorexia, photophobia, phonophobia, nausea Interventions: pain management - Avoid trigger activities - Abortive therapy: alleviating pain during the aura phase or soon after headache has started - Medications o Mild Migraines: NSAIDS, acetaminophen, Advil, Excedrin o Severe: Triptan (take as soon as headache starts), ergotamine, isometheptene (rebound headache possible) o Preventive: calcium channel blockers (verapamil), antiepileptic drugs (Topamax), antiemetics, beta blocker (pranolol, timolol) o Avoid tyramine containing products – TRIGGER - Non-drug managmenent: yoga, meditation, cool cloth, massage, exercise...
