Description

Pantoprazole Drug Name (generic and trade): Pantoprazole and Protonix Drug Classification: Proton pump inhibitor. Usual/Recommended Dose : Adult PO 40mg/day for 8wk, may repeat course. Therapeutic Action of medication:- Suppress gastric secretion by inhibiting hydrogen/potassium ATPase enzyme system in gastric parietal cell, because it blocks the final step of acid production. Common Side/Adverse Effects (Top 3): Pancreatitis, pneumonia, Myalgia, Rhabdomyolysis, blood in urine, swelling. Drug Interactions (Top 3): Plavix, Naproxen and Warfarin. Food Interactions (Top 3): Cranberry, Alcohol, and tobacco. Nursing Implications (Before giving the medication):- Ask if the patient is pregnant or breast feeding. Nursing Implications (After giving the medication):- Hyperglycemia may occur in diabetic patient, assess for severe diarrhea or abdominal pain. Patient should be educated on: Educate patients that Delayed-Release Oral Suspension and PROTONIX Delayed-Release Tablets should not be split, crushed, or chewed.

Zofran Drug Name (generic and trade): Ondansetron and Zofran. Drug Classification:- Antiemetic. Usual/Recommended Dose:- Adult PO 8mg TID, may repeat q8hrs. Therapeutic Action of medication: Preventing Nausea and vomiting by blocking serotonin peripherally, centrally and in the small intestine. Common Side/Adverse Effects (Top 3): Bronchospasm, Headache, dry mouth, Dizziness, and fatigue. Drug Interactions (Top 3): Rifampin, carbamazepine, Phenytoin. Food Interactions (Top 3): Grapefruit, St. John's Wort, poppy seeds. Nursing Implications (Before giving the medication): Check s/s of dehydration, check vitals like BP and pulse, assess for allergic reaction. Nursing Implications (After giving the medication):- Assess diarrhea, constipation or change in rate of respiration. Patient should be educated on: Take medication with full glass of water. Report any hypersensitivity reaction.

Clopidogrel Drug Name (generic and trade): Clopidogrel and Plavix Drug Classification: Platelet aggregation inhibitor. Usual/Recommended Dose: Adult PO 75mg/day with or without Aspirin. Therapeutic Action of medication: Inhibit ADP-induced platelet aggregation. Common Side/Adverse Effects (Top 3): GI bleeding, hepatic failure, Pancreatitis. Drug Interactions (Top 3):- Aspirin, Atorvastatin, warfarin, rifampin. Food Interactions (Top 3): Fish oil, Green Tea, garlic, ginger, horse chestnut Nursing Implications (Before giving the medication): Assess for thrombotic purpura, fever, neurolytic anemia, assess for symptoms of stock, lab work. Nursing Implications (After giving the medication): Assess the blood work, assess for s/s of internal bleeding, shock symptoms, Patient should be educated on: Teach patient to report any bleeding, bruising, diarrhea, skin rash, chills, sore throat....
