Oxidase Inhibitors (MAOI) CATEGORY CLASS Monamine ______________________________________________________________________ PURPOSE OF MEDICATION

Expected Pharmacological Action Increase the amount of norepinephrine, dopamine, serotonin, and tyramine in the brain. Relieves depression but increases in tyramine can cause hypertensive crisis if dietary modifications ignored.

Therapeutic Use Used for depression, bulimia nervosa, panic disorder, social anxiety disorder, generalized anxiety disorder, obsessive-compulsive disorder and posttraumatic stress disorder.

Complications CNS stimulation such as anxiety, mania, or hypomania. Orthostatic hypotension. Hypersnsive crisis, severe hypertension, headache, nausea, increased heart rate, and increased blood pressure. Local rash with transdermal forms.

Medication Administration Have phentolamine IV ready for hypertensive crisis. Inform client it can take 1-3 weeks to begin feeling effects and 2-3 months for full effects. Take meds as prescribed daily. Keep taking once effects are felt.

Contraindications/Precautions Not recommmended for pregnant or breastfeeding clients. Clients taking SSRIs and those with pheocromocytoma, heart failure, cardiovascular or derebral vascular disease, and severe renal insufficiency. Cautios use in diabetics, patients with seizure disorders and clients taking TCAs.

Interactions

Nursing Interventions Monitor BP and hold if too low. Assess for suicide risk. Educate patient on tyramine-rich foods like cheese, smoked meats, avocados, bananas, soy sauce, and red wine.

Indirect-acting sympathomimetic medications can lead to hypertensive crisis. TCAs can lead to hypertensive crisis. SSRIs can lead to serotonin syndrome. Antihypertensives have an additive hypotensive effect. Meperedine can lead to hyperpyrexia. Tyramine-rich food can cause hypertensive crisis. Vasopressors can cause hypertension. General anesthetics should not be used if a patient is on MAOIs.

Client Education Evaluation of Medication Effectiveness Improvement of mood. Increased hopefulness and will to live. Ability to perform ADLs. Improved sleeping and eating habits. Increased interaction with peers.
