The focused cardiovascular assessment is also indicated when an interval or abbreviated assessment shows a change in status from previous assessments or when new symptoms emerge.

CONSIDERATIONS

Nursing Interventions (pre, intra, post) Ask patient about Nutrtion, smoking, alcohol use, exercis, and drug use. Ask patient about HTN, heart murmurs, elevated blood cholesterol or triglycerides, congenital heart disease. Ask the patient about cardiovasular family hisory HTN, obesity, diabetes, CAD, sudden death.

Client Education Teach about the basic function of the heart. Teach the definition of heart failure Teach the patient practical tips for improving health and emotional outlook Take all medications as ordered Teach patient to reduce swelling by keeeping feet up.

Nursing Interventions Notify provider of any abnormal signs, symptoms, or abnormal findings. Use the bell of the stethoscope to auscultate bruit at the carotid artery. Use the diaphragm to auscultate heart sounds at the point of maximum impulse.
