Description of Skill Assessing client for verbal and nonverbal clues, asking if they are thinking of suicide, using SAD PERSONSassessment tool, and documenting location, mood, quoted statements & behavior every 15 minutes or per facility protocol. Indications Client is having thoughts about committing suicide; having feelings of hopelessness, helplessness and inner pain.

Outcomes/Evaluation Client develops a support-system list with specific names, agencies, and telephone numbers that the client can call in case of an emergency. Agrees to a no-suicide contract, which is a verbal or written agreement made to not harm themselves, but instead to seek help.

Potential Complications untreated depression; loss of employment and finances; powerlessness; prior attempts of suicide; change in functional ability; declining physical health; physical harm (lacerations, scratches, and scars that could indicate previous attempts at self-harm); Family Hx; physical disorders; intense emotions; access to lethal methods; lack of access to adequate mental health care.

Client Education Assist client with a no-suicide contract; it is not legally binding and should only be used per facility protocol; it can be beneficial but should not replace other prevention strategies; maintain trust between the nurse and client.
