REPUBLIC OF THE PHILIPPINES) CITY OF PASIG )S.S. AFFIDAVIT OF LOSS I, Salve Cynthia R. Monjardin, of legal age, Filipino, and a resident of Lot 13, Block 5, Molave Street, Perpetual Village 6, Mambog IV, Bacoor City Cavite, after having been duly sworn under oath, hereby depose and say: 1. That I am a subscriber of a prepaid sim card no. 0918-4266883; 2. That I lost my cellular phone when I was about to aboard a jeepney in Vicente Cruz St. corner España Boulevard last September 27, 2018; 3. That I noticed the loss when I arrived at the office on the same day; 4. That despite diligent efforts exerted to locate the same, I could no longer find, hence considered permanently lost. 5. That I am executing this affidavit to attest to the truth of the foregoing allegations for securing new sim card with the same number in lieu of the lost one.
