Selective Serotonin Reuptake Inhibitors (SSRI's) CATEGORY CLASS ______________________________________________________________________ PURPOSE OF MEDICATION

Expected Pharmacological Action Selectively inhibits the reuptake of serotonin in the CNS

Therapeutic Use Antidepressant action. Decreased behaviors associated with: panic disorder, bulimia.. Decreased mood alterations associated with PMDD.

Complications - Insomnia - Nervousness - Sexual dysfunction - Headache - Weight gain Hyponatremia (especially in older adults and those taking diuretics) Increased risk for suicidal ideation (especially children, young adults) Serotonin syndrome (elevated temp, agitation, increase reflexs, tremors, sweating, dilated pupils, diarrhea) - Saint Johns syndrome

Medication Administration - Given orally in tablets, capsules, solution, or sustained-release weekly capsules (fluoxetine). - Recognize that it may take 4 to 6 weeks to reach clinical effectiveness. Administer with food if gastrointestinal upset occurs. - Give in the morning to prevent sleep disruption

Contraindications/Precautions Hypersensitivity; Concurrent use of MAO inhibitors or MAO-like drugs (linezolid or methylene blue); Concurrent use of pimozide; Concurrent use of thioridazine (fluoxetine should be discontinued at least 5 wk before thioridazine therapy is initiated).

Interactions - Taking an SSRI within 2 weeks of MAOIs or another SSRI increases the risk of serotonin syndrome. -QT interval prolong-ing drugsmayqthe risk of QT interval prolongationwith arrhythmias; avoid concurrent use -Avoid alcohol use

Evaluation of Medication Effectiveness - Improvement of signs and symptoms of OCD, SAD, Bipolar Disorders, Depressive Disorders

Nursing Interventions Monitor for development of neuroleptic malignant syndrome (fever, respiratory distress, tachycardia, seizures, diaphoresis, arrhythmias, hypertension or hypotension, pallor, tiredness, severe muscle stiffness, loss of bladder control). Report immediately

Client Education -Look for suicidal tendencies -Instruct patient to take fluoxetine as directed. -Teach pt signs of Serotonin Syndrome -Caution pt to move positions slowly -Inform pt of proper oral hygiene -Do not stop medication abrubtly -Caution patient to wear protective clothing and use sunscreen to prevent photosensitivity reaction
