PURPOSE OF MEDICATION
Expected Pharmacological Action Produces accumulation of cyclic adenosine monophosphate at beta2-adrenergic receptors. Relatively specific for beta receptors.
Therapeutic Use Maintenance treatment to prevent bronchospasm in COPD including chronic bronchitis and emphysema
Complications headache, nervousness, palpitations, tachycardia
Cautiously used: seizure disorder, diabetes, contraindicated: Hypersensitivity to salmeterol or hyperthyroidism, milk proteins, acute attack of asthma, long-term pheochromocytoma, asthma control medications, patients whose excessive use, asthma is currently controlled on low-or-medium Cardiovascular disease dose inhaled corticosteroid therapy.
Medication Administration This medication is inhaled into the lungs
Contraindications/Precautions
Interactions Beta blockers may decrease therapeutic effects MAO inhibitors and tricyclic antidepressants potentiate cardiovascular effects. When used with caffeine-containing herbs may increase stimulant effect
Evaluation of Medication Effectiveness prevention of bronchospasm or reduction of frequency of acute asthma attacks in patients with chronic asthma. Improvement in asthma control can occur within 15 min of starting therapy, but full benefit may take 1 wk or longer. time to onset and degree of symptom relief will vary with individual. Prevention of exercise-induced asthma.
ACTIVE LEARNING TEMPLATES
Nursing Interventions assess lung sounds, pulse, and BP before and during therapy, observer for paradoxical bronchospasm. Monitor pulmonary function tests before initiating therapy and periodically during therapy
Client Education Caution patient not to use salmeterol to treat acute symptoms. A rapidacting inhaled beta-adrenergic bronchodilator shoud be used for relief of acute asthma attack.
