ASSESSMENT
SAFETY CONSIDERATIONS
Risk Factors
Expected Findings Left-sided heart failure - Dyspnea, orthopnea (shortness of breath while lying down), nocturnal dyspnea - Fatigue - Displaced apical pulse (hypertrophy) - S3 heart sound (gallop) - Pulmonary congestion (dyspnea, cough, bibasilar crackles) - Frothy sputum (can be blood-tinged)
- Left- sided heart (ventricular) failure - Right- sided heart (ventricular) failure - High- output heart failure - Cardiomyopathy (leading to heart failure)
Laboratory Tests
Diagnostic Procedures
- Human B-type natriuretic peptides (hBNP)
- Hemodynamic monitoring - Ultrasound - Transesophageal echocardiography (TEE) - Chest x-ray - ECG, cardiac enzymes, electrolytes
PATIENT-CENTERED CARE
Nursing Care - Monitor daily weight and I&O. - Assess for shortness of breath and dyspnea on exertion. - Administer oxygen as prescribed. - Monitor vital signs and hemodynamic pressures. - Position the client to maximize ventilation (high-Fowler’s). - Monitor diagnostic results to track progress.
Therapeutic Procedures - Ventricular assist device (VAD) - Heart transplantation
