Description of Skill Testing of a sample of blood to determine serum free glucose levels. Can be done bedside with a glucometer requiring only a small sample of blood or in the lab as part of a metabolic panel or a stand alone test.
Indications - Monitoring clients with diabetes - Monitoring clients on glucosteriods - Monitoring clients NPO - Monitoring clients on tube feeding. - Monitoring clients on parenteral feeding.
Outcomes/Evaluation Blood glucose levels should be between 70-100. Lower may require administration of oral glucose or IV dextrose. Higher levels may require administration of insulin.
Potential Complications - Infection of puncture site - Bleeding with coagulation comorbidities - Pain at puncture site - Inacurate readings due to edema or medications such as pressors.

CONSIDERATIONS
Nursing Interventions (pre, intra, post) pre: Check orders, verify patient with 2 point check, inform patient of test and reason for test. intra: Hand hygeine, don gloves, check for uncallused site for testing, clean surface with alcohol pad, puncture with lancet, squeeze to express a drop of blood and wipe away, squeeze second drop, touch test strip to drop until sample is read. post: apply pressure with gauze until bleeding stops, dock glucometer and verify reading has populated, check orders for medications to administer corresponding to blood glucose reading and admionister.
Client Education - Educate on hygeine - Educate on necessity of regular checks per prescriber orders - Educate on use of equipment - Educate on procedure - Educate on checking test strip expiration dates
Nursing Interventions - Daily Calibration of meter - Check test strip for expiration - Administer glucose or dextrose for readings below range - Administer proper insulin(s) for readings over range. - Retest if out of range with lab specimen.
