B. Write your community health nursing diagnosis statement. Obesity in the pediatric population: There is an increased risk of obesity among the pediatric population within Pasco County, Florida related to low physical activity and unhealthy diets in relation to the current pandemic as evidenced by an increasing obesity rate. 1. Explain how the health concern from your community health nursing diagnostic statement is linked to a health inequity or health disparity within the target population. Obesity is among one of the greatest public health concerns in the United States, with one in five children being obese. This equates to approximately 340 million children and adolescents who meet the criteria based on 2016 data [ CITATION Che20 \l 1033 ]. In my community, there are multiple class levels, ranging from poverty to wealth, but the overall income is on par with a middle-class income. Upon research, we have a high number of individuals who do not have adequate health insurance, if at all, and many are receiving Medicaid or Medicare. Due to the current pandemic, there has been a noticeable increase in the cost of food, among other things, and this creates a concern that parents are finding it harder to locate and purchase healthy food options for their family, as processed foods, cookies, and fast food comes in at a lower cost than the fresh fruits, vegetables, and unprocessed foods. Upon assessment of several grocery sites, I noticed that people were spending much of their shopping time in center isles, rather than the outer isles that contain the fresh fruits and vegetables. Most shopping carts contained pasta, frozen meals, and canned vegetables, there were carts that incorporated few fresh fruits and vegetables.

3 Upon assessment of several gyms and exercise locations, I observed a healthy mix of people, those that were well in shape, and those who may have just begun their journey at the gym. The cost for the gyms varied, along with the type of equipment, classes, programs, and amenities offered. Several of the fitness facilities offered youth programs and classes. Most offered an after-school program and a summer camp option. I noticed the cost of these programs are more on the expensive side where many families may not be able to afford these programs, especially if they have multiple children. If a family does not have enough money for healthy food, they more than likely will not be able to afford a costly program for their child/children. a. Discuss the primary community resources and primary prevention resources currently in place to address the health concern. There are a few resources in my community available to individuals who would like to eat healthier. There are only two local food banks, and they give out food on a first come first served basis. I spoke with a volunteer at Harper Valley Son’s Food Pantry to understand how their service works. They are open from 9am to 1pm every Thursday and close earlier if they run out of food to hand out. Some of the food may be nearing expiration and some may be dented or misshapen but are still fresh and couldn't be sold in stores. My community also participates in the WIC program for Women, Infants, and Children, and they are a federally funded supplemental nutrition program. WIC provides an EBT card to purchase healthy foods, such as fruits, vegetables, milk, whole grains, eggs, cheese, juice, and cereal. They also offer nutritional education and counseling, breastfeeding support, and referrals for other community and healthcare services. There are criteria that must be met in order to qualify, which include some of the following: being pregnant, had a baby within the last 6 months, breastfeeding mother, being a child or foster child under 5. WIC is also based upon

4 income, if the child is on Medicaid or they are receiving any SSI income. There is an online guideline tool that can be used to see if someone qualifies, but an appointment is necessary for the WIC employee to determine if you qualify or not [CITATION WIC22 \l 1033 ]. My community has several parks and recreation centers that have outside walking trails, playground equipment, running tracks, and outdoor workout equipment. Parks and recreation centers outdoor areas are free and open to the public from dawn until dusk, weather permitting. When assessing these places, I noticed a few families partaking in the outdoor exercise equipment together and some children playing on the playground. Florida has nice weather for most of the year, so I was a little surprised when I didn’t see very many people at the parks, as these are great places for families to visit during the current pandemic as social isolation is possible. b. Discuss the underlying causes of the health concern. The underlying causes may be attributed to not having enough money for both food and exercise, and there may not be enough knowledge on daily food intake and the amounts that are appropriate, as well as how to properly read nutritional labels on food. According to the U.S. Census Bureau, 11.3% of residents in Pasco County Florida are living at or below poverty level [ CITATION Uni20 \l 1033 ]. When dining out, restaurants provide portions large enough for two or more people to share, therefore people are overeating, taking larger portions than recommended, not getting enough fruits, vegetables, healthy fats, etc. Without proper knowledge and education, obesity is a problem that will not be going away. Although physical education is offered in school as part of an educational curriculum, it is limited to two days a week and does not always entail exercise, some days may be spent inside

5 if there is inclement weather. In Pasco County, Florida, recess is limited to 30 minutes per day but is not required to be outside, rather just 30 minutes of free time. 2. Discuss the evidence-based practice associated with the Field Experience topic. Healthy People 2020 states that “diet and body weight are related to health status”[ CITATION Hea21 \l 1033 ]. Evidence has proven that there are health benefits of eating a healthy diet and maintaining a healthy body weight. Risk of diseases such as diabetes, hypertension, heart disease, obesity, and some cancers can be reduced by maintaining a healthy lifestyle. Obesity has more than doubled among children 6-11years and tripled among adolescents 12-19 years [ CITATION Jan20 \l 1033 ] There are numerous gyms, health facilities, and food banks within my community that can help aid in the prevention and treatment of obesity. It is up to the members of the community to take the proper steps to help themselves. People may need to be educated on the resources available within the community to better take advantage of them. a. Identify data about the selected Field Experience topic from the local (e.g., county), state, and/or national level. According to the Florida Department of Health, approximately 66% of adults and 27.4% of children are overweight or obese [ CITATION Flo19 \l 1033 ]. If we continue to follow our current trend, almost 60% of our population will be obese by 2030, costing an estimated $34 billion in preventable chronic diseases. C. Develop a community health nursing social media campaign strategy that will convey your health message and address the Field Experience topic by doing the following: 1. Describe your social media campaign objective.

6 The objective for my social media campaign will be to raise awareness of the available resources my community has in order to lower the current obesity problem in Pasco County. 2. Recommend two population-focused social marketing interventions and justify how each would improve the health message related to your selected Field Experience topic. The first social marketing intervention I would utilize would be Instagram. More and more people have moved over to Instagram from other social media platforms so I feel this would reach a larger audience. I would start off by creating a campaign that highlights the local food pantries, WIC services, and youth fitness programs offered by the local community centers. The second intervention I use would be YouTube videos. It is child friendly and would allow children to view healthy recipes for all diets, including vegetarian and vegan, easy at home exercises that they can do indoors or outdoors, and teach them about daily nutritional values. These videos would also educate them on how to properly read a nutrition label so they can make informed nutritional choices. 3. Describe a social media platform you would use that is appropriate for communicating with the target population. YouTube is used by most people on a regular basis. They have videos for just about thing someone would need to view. This program will be mainly directed towards children and teenagers but could easily be utilized by adults as well. Children wouldn’t need to set up an account, so if they know how to use the internet, they can view the videos. If they do not know how to use the internet their parents can help them to view these videos. Weekly exercise and meal menu videos would be uploaded to ensure content is fresh and relevant to the current needs of the community.

a. Discuss the benefits of the selected social media platform in supporting preventative healthcare. The use of my social media campaign will help to promote a healthier community by having access to a multitude of recipes, exercise routines, local community resources available without having to leave their home. An Instagram page will promote current community activities happening daily or weekly, current food bank information, and how to reach out to local services that are available to them. YouTube videos will be uploaded weekly so that families can meal plan, allowing for a more successful healthy lifestyle. Registered dieticians would be utilized for nutritional information, a local chef would be utilized for easy, healthy recipes that are also budget friendly, and a local personal trainer would be utilized for easy, at home exercises that do not require a lot of equipment to be successful. 4. Discuss how the target population will benefit from your health message. This campaign will be beneficial to those who may feel like they don’t have the resources to meal plan or develop a current exercise routine. People that do not like to leave their home to exercise will feel comfortable in their own home while becoming healthy. This social media campaign could be the start that the community needs to become healthier and break the current cycle of obesity. If the campaign had great response and success in Pasco County, it would be a good idea to expand to neighboring counties as well. D. Describe best practices for implementing social media tools for health marketing. Setting reasonable and attainable goals is one of the best strategies for a social media campaign. You should offer information that is truthful and correct. My social media campaign

8 will be very straightforward and worded accurately for my target audience. Too many posts or videos with too much detail or data could likely cause loss of interest in the program. Color schemes and fonts should be appealing to the eye as to not deter viewers. E. Create a social media campaign implementation plan by doing the following: 1. Describe stakeholder roles and responsibilities in implementing the plan. Stakeholders for this social media campaign would be Instagram, YouTube, local WIC office, food banks, local gyms and fitness centers, local parks and recreation centers, teachers, physical education instructors, myself, and local youth programs that would be interested in coming on board to help develop these campaigns. Our targeted audience would be another very important stakeholder as they are the ones that will provide feedback on the campaigns, allowing for any changes to be made that would better suit the audience. 2. Discuss potential public and private partnerships that could be formed to aid in the implementation of your campaign. School districts could become involved by providing healthier food options for each meal and snack and ensuring that every child has a nutritious meal. Posters with the Instagram and YouTube advertisements for the campaign could be displayed in the cafeteria and flyers could be sent home with the children. The schools could also incorporate the exercises in their daily school schedules, and maybe the children will utilize the exercises at home. Having the schools involved will be a great impact for the success of the campaign. 3. Create a specific timeline for implementing your campaign. This campaign could easily be set up and going within a week or two. YouTube would only require a channel to be set up. Recording of the videos would take about a day or two for

9 the first week’s content. Instagram would require an account to be set up and then content can be created daily to post. 4. Explain how you will evaluate the effectiveness of the campaign. The effectiveness of this campaign can be evaluated by the number of participants who are actively engaging in these programs, the number of times the videos are viewed, and the response to the content posted on each platform. Surveys could be sent out to registered viewers on the platforms, asking for feedback, comments, and recommendations for each program. Both of these options will allow us to monitor the success of each campaign. 5. Discuss the costs of implementing your campaign. One major benefit of utilizing social media for campaigns, is the relatively low to no cost. Both platforms chosen are free to use and since we would not be advertising on these sites, there wouldn’t be an additional fee. Local businesses in health improvement, such as gyms and grocery stores, could donate money or supplies for printing materials for the families, which can then be used as a donation write-off on their taxes and a volunteer could run the Instagram and YouTube pages. F. Reflect on how social media marketing supports the community health nurse’s efforts to promote healthier populations. Though the role of the community health nurse has changed over the years, the goal remains the same, to help improve the overall health of the community and reduce chronic disease. Technology is ever expanding and growing so social media is the perfect platform to reach the community. By utilizing a platform that is popular with the target age group, our campaign would encourage interaction and could lead to a healthier community.

10 1.Reflect on how your social media campaign could apply to your future nursing practice. As a pediatric nurse, I believe these two campaigns could easily be used in any healthcare setting. The YouTube videos can be utilized on patient televisions for their current ordered diet in an inpatient setting. Healthy recipe videos can be played in the pediatrician’s office, along with printed recipes of the week that parent can take home. We can easily help patients register and start using the platforms while they are with us inpatient. Making a game out of the daily exercises can be a fun bonding moment with the nurse and patient and can lead to continued use at home. Educating patients is a passion of mine so I feel that any resources we offer is a success.
