Description of Procedure *Ultrasound to see physical and physiologic signs of the fetus in utero and watch for fetal response to stimuli. This is combined with Fetal Heart Rate Monitoring (FHR).

Indications * Diagnostic test for fetal wellbeing * Potential diagnosis include: * Nonreactive stress test (NST) * Oligohydramnios (No enough amniotic fluid) * Polyhydramnios (Too much amniotic fluid) * Fetal hypoxemia (Low oxygen in blood) * Fetal Hypoxia (Not enough oxygen in blood)

Interpretation of Findings * Assesses fetal well being using 5 variables; each worth two points, to a max score of 10 (each has 30 min and is referencing fetus) * It is either 2 points for each category or 0 * FHR (Reactive NST = 2 points) * Breathing Movements (1 episode of 30 seconds or more = 2pts) * Body Movement (3 body/limb extensions & flexion return = 2pts) * Fetal Tone (1 episode of extension with return to flexion = 2pts) * Amniotic fluid (1 pocket of fluid that measures 2cm in 2 perpendicular planes = 2 points) Scores: * Normal 8-10 * Abnormal 4-6, suspect chronic fetal asphyxia * Abnormal 0-2 - strongly suspect chronic fetal asphyxia

Potential Complications * Supine Hypertension during the procedure: * When the client is on back the weight of the uterus and fetus can compress the vena cava and decrease blood flow and oxygen to the uterus and fetus. Client may experience lightheadedness and fainting.

CONSIDERATIONS

Nursing Interventions (pre, intra, post) Pre: * prepare client as you would for an ultrasound * client should have a full bladder, recommend to drink 1 quart of water prior to the procedure. * Supine with a wedge under the hip to displace the uterus and prevent hypotension. Intra: * Use warm gel on the clients abdomen (NOT HOT) Post: * Allow client to empty bladder * Clean gel off abdomen

Client Education * Fetal and Maternal structures can be pointed out to the client as the ultrasound procedure is preformed * Instruct client to drink 1 quart of water prior to the procedure

Nursing Interventions * When the client is supine, place a wedge under the clients hip to displace the uterus and prevent hypotension. May need to move the client to more of a side-lying position.
