PURPOSE OF MEDICATION

Expected Pharmacological Action Acts on alpha and beta receptors. This nonselective adrenergic agonist stimulates: postsynaptic alpha2 receptors, which constricts arteries.

Therapeutic Use To treat cardiac arrest.

Complications CNS> Anxiety, apprehensiveness, chills, disorientation CV> Arrhythmias, including ventricular fibrillation GI> Anorexia, heartburn, nausea, vomiting

Contraindications/Precautions Epinephrine is contraindicated in patients with cerebral arteriosclerosis, coronary insufficiency, dilated cardiomyopathy, general anesthesia with halogenated hydrocarbons or cyclopropane, sensitivity to epinephrine.

Interactions resuscitation Rapid acting vasodilators: Blockage of epinephrine’s alpha-adrenergic effect, possibly causing severe hypotension and tachycardia. Antihypertensives: Decreased antihypertensive effect

Evaluation of Medication Effectiveness Improved perfusion AEB urine output greater than 30ml,hr with adequate kidney function and improved mental status. Blood pressure maintained within normal limits

Nursing Interventions Use epinephrine cautiously in patients with angina, arrhythmias, asthma, degenerative heart disease or emphysema. Epinephrine’s inotropic effect equals that of dopamine and dobutamine;

Client Education Warn patient not to exceed recommended dosage or to shorten interval because of the risk of adverse reactions and tolerance
