Success of the Performance Improvement Plan

Financial Implications According to Rhonda Collins from Becker Review Hospital she states “according to a survey by the Risk Management Foundation of the Harvard Medical Institute, communication failures in the U.S. hospitals and medical practices were responsible for at least 30 percent of all malpractice claims, resulting in 1,744 deaths and $1.7 billion in malpractice costs over five years.” (Collins, 2017). For team meetings (huddles) and communication training to be successful it would have to involve all healthcare facility department staff. Doctors, Case Managers, Social Workers, CNA’s, Nurses, Therapist and Management would be included as those that need to understand and go for these meetings and communication training in order to better benefit the facility and themselves. It is crucial for everyone to work together in order to improve communication and increase the success of patient handoff. If the initiative is deemed successful, then financial suggestions for healthcare facilities will improve drastically. By having meetings (huddles) and communication training given from providers all staff members would be able to learn better methods in order to change the communication issues and have effective patient hand-offs. These options of having meetings and training for communication skills would greatly improve patient care, safety, quality, and satisfaction, and lower the numbers of communication failures and malpractices issues.

Existing Information Management Contributions Information Management Systems can improve patient communication and patient handoffs, this can be serious if it isn’t addressed properly and following all criteria of The Joint Commissions regulations. Using electronic medical records or health records it can help in improving the success of communication and patient hand-offs, however it isn’t always as reliable as many mistakes can happen. EMR/EHRs can’t always be relied on because it could be assumed that staff members and providers are reading notes in the record but not following up appropriately. Information Management Systems can help provide success in both daily meetings, communication issues, and patient hand offs is they can make templates that gather data on patient care. By having the patient data collected and documented this can help contribute to the success of improving communication skills and effective patient hand-off.

Organizational Process that Permit Continued Viability Electronic health records (EHRs) or electronic medical records (EMRs) and other technologies this can help improve the hand-off issues with patients and providers. Information Management System should be used by having physical and visual communication and patient hand-offs. To have the most effective communication and patient hand-offs they need a free area where they won’t receive any interruptions or interferences when they are talking or having patient hand-offs. This can help improve the use of EHRs or EMRs when they can find a designated area to communicate and handle patient hand-offs. The Joint Commission always stresses the organizational and cultural aspect of improve hand-off communication. Some of the suggestions that they made to improve communication and hand-offs are: demonstrating leadership’s commitment, managing the environment to provide locations free from

interruptions, including multidisciplinary team members and the patient and family, standardizing training, monitoring the success of interventions, and Sustaining and spreading best practices (Collins, 2017). Following these should help to improve the communication skills and patient hand-offs and these can be deemed successful if they are followed appropriately.

Interdepartmental Communication Patterns Necessary For communication and effective patient hand-off to be successful there are interdepartmental communication patterns that need to be used. It is important that there is frequent training for all healthcare staff members. To have team meetings (huddles) and communication training be successful all members of the healthcare facility need to be involved in these training and meetings to be sure of the importance of having these followed. In any organization including healthcare it is important for every staff member to work together in order to improve communication and patient hand-offs. There should be a mandatory and regular training regarding patient hand-offs and communicating appropriately and should be in all healthcare organizations. After training is applied as a standard then healthcare managers will perform regular check-ins and send reminders to be sure that healthcare providers will follow these standards so that they can improve communication and patient hand-offs. Interdepartmental communication should be used to offer support and continue training so that patient care keeps improving patient safety and satisfaction.
