Expected Pharmacological Action May reduce blood pressure by inhbiting conversion of angiotensin2. Lisinopril may also inhbit renal and vascular production of angiotensin2

Therapeutic Use To improve survival in hemodynamically stable patient within 24hrs of an acute MI.

Complications CNS Ataxia, confusion, depression, dizziness, fatigue, hallucinationsC Arrythmias, chest pain, fluid overload, hypotensionGI Abdominal pain, anorexia, constipation, diarrhea

Medication Administration 5mg within 24hr after onset of symptoms, followed by 5mg after 24hrs and 10mg after 48hr. Maintenance: 10mg daily for 6 weeks.

Contraindications/Precautions Concurrent aliskiren use in patients with diabetesor patients with renal impairment; hereditary or idiopathic angioedema or history of angioedemarelated to previos treatment with ACE inhibitor; hypersensitivity to lisinopril, or other ACE inhibitors, or their components.

Interactions ACE inhibitors: Increased risk of hypotension, hyperkalemia, and renal impairmentallopurinol, systemic corticosteroids: Increased risk of potentially fatal agranulocytosis or neutropenia.

Nursing Interventions Be aware that lisinopril should not be given to a patient who is hemodynamically unstable after an acute MI.Use cautiously in patients with FVE, heart failure, impaired renal lfunction, or Na+ deficit

Client Education Evaluation of Medication Effectiveness Successful reduction in blood pressure with eventual maintenance at range levels
