CONCEPT MAP/ PLAN

OF

CARE

Est. Time: 30 Minutes

This activity creates an opportunity for you to organize the nursing care required for the patient care presented in your assigned vSim. S TUDENT L EARNING OUTCOMES At the end of this activity, student will be able to: 1.

Describe pathological events associated with the patient’s disease process or condition.

2.

Create a plan of care and prioritized nursing interventions based on patient care needs.

3.

Identify anticipated diagnostic and physical assessment findings related to the identified condition or disease process.

ASSIGNMENT 1.

Log into thePoint and launch the assigned vSim.

2.

Review the information contained in the patient information.

3.

Review the smart sense links associated with Nursing Care, Diagnostics, and Pharmacology found in the suggested reading area.

4.

Create the following “concept map”. List the pathophysiology associated with the patient’s disease process or condition, the anticipated physical assessment findings, vital signs, diagnostics, specific nursing interventions, and other patient information associated with the patient situation.

5.

Utilize the smart sense links throughout the vSim to complete the worksheet.

6.

Submit your concept map in Blackboard as instructed.

vSim for CONCEPT MAP WORKSHEET DESCRIBE DISEASE PROCESS AFFECTING PATIENT (Include Pathophysiology of Disease Process) Hip arthroplasty is a surgical procedure in which there is removal of the damaged area or surface of the femoral head and potential damaged cartilage. In a femoral head fracture, the shear force against the head of the femur as it leaves the acetabulum (socket) is the thought to why it causes the femoral head to fracture when it is dislocated. A hip arthroplasty is an alternative to a total hip replacement. It is a reconstructive surgery where a prosthetic cap is added to the femoral head and a fixated cup is added to the socket which causes relocation and stabilization of the femur back into the hip socket. It is a procedure that is contraindicated in some instances such as patients with severe fractures, patients with impaired kidney function, and patients with arthritis, and in some older adults. It requires work with physical therapy and rehabilitation.

Blood transfusion reactions: There are a variety of blood transfusion reactions that can occur when a patient undergoes a blood transfusion. It has several different manifestations depending on what type of transfusion reaction occurring. These manifestations range from chills, fever, shortness of breath, hives, and back pain. The different types of blood transfusions reactions include acute hemolytic, allergic, febrile, and bacterial. It is important to monitor and educate the patient about the signs/symptoms of transfusion reactions before infusing ANTICIPATED PHYSICAL DIAGNOSTIC TESTS PATIENT INFORMATION (Reason for Test and Results) FINDING Cross-matching of blood: to make  Back pain Name: Lloyd Bennett sure blood types match (safety for  Tenderness of wound or DOB: 10/28/1945 patient) around wound when Gender: Male Religion: N/A palpated Urinalysis: assess kidney Age: 76 y/o  Redness around incision Ethnicity: N/A site functioning Weight: 75 kg  Slight edema around Code status: full incision site Venous blood samples: to check electrolytes and important Height: 180 cm  Sweating, fever/chills Allergies: NKDA  Pain related to surgical markers in blood concerning amount of blood and coagulation Diet: Regular incision, increased when Provider: William Smith, MD  PTT hip is flexed or during  Hemoglobin and Reason for admission: Left femoral ambulation Hematocrit head fracture, left hip arthroplasty  Shortness of breath X-ray: to observe place of fracture
