ATI: Health Assess Virtual Practice Harold Stevens (Cardiovascular) (Under the apply tab of ATI. Go to Health Assess.) Objectives: After completion of the lesson, the learner will be able to virtually:  Gather tools needed for a focused assessment on a client with a cardiovascular health alteration.  Conduct a focused interview with a client with a cardiovascular health alteration.  Apply the techniques used to perform a focused assessment on a client with a cardiovascular health alteration.  Differentiate between normal and abnormal findings on a client with a cardiovascular health alteration.  Document assessment finding in the electronic health record. Tasks:  Pre-virtual Simulation activity A. Complete the Cardiovascular learning module under Health Assess module before completing the Virtual Practice Harold Stevens.  a. Listen to and view all screens in the learning module.  b. Review the animations, videos, and images in the learning module.  c. Review glossary items as needed including proper pronunciation of key terms. B. Review Introduction to Health Assessment: Inspection, Palpation, and Auscultation C. Read the overview for Virtual Practice: Harold Stevens D. Complete the following Pre-virtual Simulation Day Questions on a separate sheet of paper to be turned in later: 1. What tools did you obtain prior to beginning the focused assessment on a client with a cardiovascular health alteration? - Stethoscope to listen to the heart and lungs, Penlight to see if pupils are round and reactive,

Watch to count apical pulse for a full minute. BP Cuff to measure their blood pressure. 2. What questions are you going to ask the client? -What brought his in today? -Is he allergic to any medications? -Do you smoke of drink if so, how much per week? Or when did he give it up? -Does anyone else have a history of this certain problem in his family? 3. What physical assessment activities do you anticipate performing? - A full head to toe assessment and assessing what his skin looks and feels like. Making note of what his heart, lungs, and abdomen sound like. Taking his VS to obtain a baseline. 4. What type of information will you chart in the EHR? - Demographic information, personal and family medical history, allergies, immunizations, medications, health conditions, vital signs, contact and insurance information, as well as a record of what has occurred during visits with the provider.  Virtual Practice: Harold Stevens

Post-Virtual Simulation Activity A. Complete the following questions on a separate sheet of paper: 1. What did you learn from this scenario?

- I learned what assessment procedures are necessary for a patient with heart failure compared to a healthy patient and how important it is to focus on the cardiovascular and pulmonary systems. 2. What was the client’s main concern? - The clients main concern was his heart failure. Checking his respiratory effort and his heart was very important to make sure he was breathing correctly and getting the correct amount of oxygen. 3. Did you uncover any unexpected subjective or objective data? -He becomes SOB due to any activity -He is tired all the time regardless of how much sleep he gets -He isn’t in any pain -He does not have a cough. 4. What helped you recognize that these findings were unexpected? - Lung sounds are supposed to be clear; heart sounds are only supposed to be S1 and S2, edema is an irregular finding, becoming short of breath that easily isn’t normal either. 5. What factors during the scenario influenced the questions you asked the client? - The fact that the patient was short of breath could indicate him having a cough or his fatigue. Asking him if he was in pain came from his VS and needing to know, and because he has an underlying disease that could cause pain. 6. What factors during the scenario influenced the actions you took during the physical assessment? - The subjective data that the patient gave me and the patients charting that the patient had hypertension and heart failure made me focus in more detail on certain assessment interventions and body systems. 7. Are there additional questions you would ask the client if this type of scenario happens again?

-How is your stress level? -Are you still having a hard time catching your breath? -How often are you exercising? -Do you ever feel your heart racing? 8. Are there additional actions you would take if this type of scenario happens again? - Look for weight increasing, check I O daily to make sure the patient isn’t experiencing fluid volume overload, assess edema, examine patient’s neck and for any vein distention, look for ascites, do a chest xray.
