Description

Predisposing Factors 1. Suspected sleep apnea 2. Acute on chronic combined systolic and diastolic CHF 3. Pneumonia 4. Morbid obesity 6. Hx of smoking

Precipitating Factors:

CHRONIC OBSTRUCTIVE PULMONARY DISEASE

SOB occurs at rest or exertion

Pathophysiology

Nursing Diagnosis Ineffective Airway Clearance r/t smoking AEB by dyspnea, productive cough, and wheezes

Hx of Tobacco Use Inflammation of central airways

Goals

Medications 1. Atorvastatin 2. Cardevidol

Destruction and disrupts the normal defense mechanisms and repair process of the lung

1. 2.

Oxidants (cigarette smoke) inactivate antiproteases (prevent natural destruction of the lungs) stimulate secretion and ↑ fluid in lungs

Pt will demonstrate the proper technique of pursed lip breathing effectively by the end of the teaching session. Pt will demonstrate effective coughing and clear breath sounds

3. Chlorthalidone 5. guaiFENesin

Nursing Interventions Vessels thickened, inflammatory cells infiltrate, collagen deposit, and capillary bed destruction

1.

6. Isosorbide mononitrate Airflow is trapped in the smaller airways 7. Ranolazine Hyperinflation of lungs

Labs: ↑ Mg

Loss of elastic recoil Excess mucus production

Alveoli destruction Bullae and blebs formation Gas exchange abnormalities Hypoxemia + hypercapnia

2. 3.

4.

Support client in using pursed-lip breathing and controlled breathing techniques. It improves respiration function and relieves dyspnea. Perform relaxation techniques. Reduce dyspnea and anxiety. Monitor respiratory rate and signs of wheezes, irregular breathing, and dyspnea. Reposition the patient into a semi/HighFowler’s position. Promotes better lung expansion and improve air exchange

↑ Glucose EKG:

Pulmonary HTN

· Left ventricular hypertrophy · Mild to moderate degree of aortic stenosis · Mitral annular calcification XR Chest View

·

atelectasis

Evaluation Hypertrophy of the right ventricle Right-sided HF
