Expected Pharmacological Action Potentiates the inhibitory effect of antithrombin on factor Xa and thrombin.

Therapeutic Use Prevention of thrombus formation

Complications confusion, CVA, epidural/spinal hematoma, paralysis, a-fib, CHF, peripheral edema, thrombosis, colestatic and hepatocellular liver injury, hematemesis, melena, hemorrhage, HIT/immune-mediated thrombocytopenia, purpura, thrombocytopenia, pulmonary edema/embolism, anaphylaxis, shock, hyperkalemia

Medication Administration 30mg subQ 12 hrs This route, frequency, and dosage is used to prevent DVT after knee or hip replacement and for prophylaxis after hospitalization for hip replacement Typically given every 12hr starting 12-24hrs after surgery for up to 14 days

Contraindications/Precautions Hypersensitivity to benzyl alcohol, positive in vitro test for antiplatelet antibody in the presence of enoxaparin; active, major bleeding, severe hepatic/renal impairment, severe uncontrolled hypertension, bacterial endocarditis, bleeding disorders, spinal/epidural anesthesia/puncture

Interactions NSAIDs, oral anticoagulants, platelet aggregation inhibitors, thrombolytics

Nursing Interventions expect to give drug with aspirin to pts. with unstable angina, STEMI, and non-Q-wave MI Watch closely for bleeding and notify provider if platelet count falls below 100,000 Test stool for occult blood keep protamine sulfate nearby in case of overdose Monitor serum potassium level for elevation

Client Education Evaluation of Medication Effectiveness Decreased or absent thrombus formation
