Xanthine Oxidase Inhibitors

Therapeutc Effects/Uses Lowers serum uric acid levels. Prevention of attack of gouty arthritis and nephropathy. Treatment of secondary hyperuricemia. Contraindicatons Hypersensitivity.

Usual Adult Dose/Route 100 mg/day, increase weekly based on serum uric acid levels.

Acton of Drug Stops the action of xanthine oxidase therefore inhibiting production of uric acid.

Side Effects/Adverse Reactons Hypotension, hypertension, bradycardia, heart failure, drowsiness, diarrhea, hepatitis, nausea, vomiting, renal failure, hematuria, urticaria, bone marrow depression.

Nursing Implicatons  Why is this drug prescribed for my patent? To lower serum uric acid levels, manage gout.  Ordered Dose: 100 mg tablet PO daily

Drug-Lab-Food Interactons Do not use in bone marrow suppressant drugs. If used with amoxicillin or ampicillin, patient is at increased risk of rash. Increases effects of warfarin. Increased risk of hypersensitivity with ACE inhibitors. Patent/Family Teaching Take as directed, do not skip doses. If a dose is missed, take as soon as remembered. Take with NSAID during a gout attack. Alkaline diet may be ordered. Can cause drowsiness. If patient notices chills, fever, skin rash, blood in urine, or nausea, contact provider immediately.

 Assessments  Lab Tests Monitor intake and output ratios. Assess patient Serum and urine uric acid levels. for rash or skin reaction. Monitor for joint pain and swelling.  Potental Nursing Diagnosis Acute Pain  Interventons Watch dosage in patients with renal failure.

 Evaluaton Decreased urinary uric acid levels and serum. Decreased symptoms of gout.
