Indications: Sepsis: A fever > 101ºF (38ºC) or < 96.8(36ºC) HR > 90 be, R > 20, probable or confirmed infection Severe sepsis: Decrease urination, changes in mental ability, low platelet, trouble of breathing cold due to fall in body temperature and unconsciousness Septic shock: Include the symptoms of severe sepsis, plus a very low blood pressure.

SEPSIS

Concept: Sepsis is a serious medical condition cause by the body response to the complicated of symmetric infection. Sepsis develops when the chemicals the immune system releases into the bloodstream to fight an infection cause inflammation throughout the entire body instead. Sepsis can cause tissues damage, organs failure, and death.

Assessments: Blood culture test. Assess patient vitals sign for higher or lower than normal, such as fever or clammy, low blood pressure, increase heart rate, difficult of breathing and any sign of infection, Assess patient mental status for confusion, sleepy, difficult to rouse.

Interventions: Monitor vital signs (RR, P, O2, B/P, Temp, LOC). Blood culture test to indicate pathogen. Monitor I&O Admitted fluid as prescribed. Admitted antibiotic as prescribed. Treating the source of the infection

Complications: Sepsis can cause tissues damage, organs failure, and death if delay in treatment....
