MY BIG IDEAS (LEARNING ESSENTIALS) As a future teacher, I need to know and accomplish the various school forms properly, accurately and legibly. It is expected that I am able to follow the Revised Guidelines on the Preparation and Checking of DepEd Forms (Div. Memo. No. 29 s. 2013)

IMPORTANCE OF THESE SCHOOL FORMS

This provides detailed information per learner of the summary data being reported to EBEIS (GESP/GSSP). This serves as the daily updating of students’ attendance and to monitor absenteeism. To monitor all textbook being used which should be reported regardless of number of books per subject area. It is lifted from SF 2 submitted by teacher/adviser. Division office furnishes a copy of this report for the months of June (attachment to GESP/GSSP-BoSY) October and March (attachment to GESP/GSSP-EoSY). This is used for all grade/year levels regardless of curriculum program. Specific instructions shall be observed in harmony with the implementation of K to 12 program. This will used for both public elementary and secondary schools. This is be submitted to the Division Office as attachment of GESP/GSSP BoSY. MY ANALYSIS (LEARNING EPISODE DIRECTIONS) A. WHY DO I NEED TO PREPARE THE DEPED SCHOOL FORMS PROPERLY, ACCURATELY AND LEGIBLY? I need to prepare the DepEd school forms properly, accurately and legibly because to these were the important records of the students. In School Register (Form 1) for example, I should write it accurately and properly to avoid voids with regards to legal matters if so. For Class Records, I should write it legibly to accurately assess all the scores without any mistakes.

MY REFLECTION / MY INSIGHTS A. GOING OVER THE VARIOUS SCHOOL FORMS, I HAVE LEARNED THAT: Going over with the various school forms, I have learned that being a teacher involves lots and lots of documents. You make sure you have the details of each of your students and you accurately record ALL of their scores.
