4. What is a combining form? Combining form is a word root to which a vowel has been added. A combining vowel (a, e, I, o, u and sometimes y) links the root to the suffix or the word root to another root. 5. Explain the purpose of the medical record, and describe three of its components. Medical records are known as electronic health records (EHR) it is an electronic record of health-related information for an individual that is created, gathered, managed, and consulted by authorized healthcare clinicans and staff. This system is used to make communication among physicians and staff members better so that everyone can be on the same page and have a better understanding of what each inidivual patient needs. There are many general components of patient’s medical record however, three of the components include: patient data, medical history (Hx), physical examination (PE). 1. Patient Data is information that is provided by the patient and then updated as necessary. This is data that relates directly to the patient, including last name, first name, DOB, gender, address, etc,. 2. Medical History: is documents describing past and current history of all medical conditions experienced by the patient. 3. Physical Examination: is records that include a current head-to-toe assessment of that patient’s physical condition. 6. What is a SOAP note, and what are its components? The SOAP chart note is a method of documentation employed by healthcare providers to write out notes in a patient’s chart, along with other common formats, such as the admission note. The SOAP note is a method of displaying patient data in a concise, organized format and is written to improve communication among those caring for the patient. The four components to the SOAP note are Subjective, Objective, Assessment, Plan. 1. Subjective: Chief complaint, history of present illness, history, review of systems 2. Objective: Vital signs =, physical exam findings, laboratory data, imaging results, etc. 3. Assessment: Interpretation of the subjectiveand objective findings. A diagnosis. 4. Plan: includes the management and treatment regimen for the patient.
